We offer a Preferred Provider Organization (PPO) plan to all eligible employees (MD employees also have the option of choosing an HMO plan). Both the HMO and PPO offer employees a network of physicians to choose from and cover routine services, major medical, hospitalization, and prescription drug benefits.
Preferred Provider Organization (PPO) allows eligible employees to choose from a large network of dentists to receive in-network benefits.
The vision plan provides the following services for eligible employees: eye exam, contacts, frames, and discounts on laser eye surgery and prescription glasses.
Health Advocate Services
Health Advocate is a service designed to ensure that employees and their family receive enhanced personal care and attention regarding complex or confusing healthcare issues. As an employee of JCVI, employees and their family will automatically have access to Health Advocate â€”Â free of charge! Their trained staff will assist with a wide range of medical and administrative issues.
Group Life Insurance and Accidental Death & Dismemberment
This benefit is 100% paid for eligible employees. The benefit pays up to 5-times an employee's annual salary up to a maximum of $500,000.
Supplemental Life Insurance
Employees are eligible to apply for additional life insurance for self, spouse, and/or dependents.
Domestic Partner Benefits
Offered for medical, dental, and vision insurance plans. Also offered for Supplemental Life Insurance.
Long-Term Care Insurance
This benefit gives employees and their family some financial protection from the costs of long-term care expenses. A long-term care "starter plan" benefits is provided at no cost to the employee. Employees also have the option to purchase additional coverage.
Business Travel Accident Plan
When on business travel, employees receive coverage equal to 5 times their annual salary up to a maximum of $900,000. JCVI pays 100% of the cost.
Short Term Disability
Coverage will provide employees with a tax-free benefit equal to 60% of their salary if a disability prevents them from working for 8 or more days. This benefit is 100% paid for eligible employees.
Long Term Disability
This benefit is 100% paid for eligible employees. The tax-free benefit will provide employees with 60% of their salary if a disability prevents them from working for 91 or more days.
Employee Assistance Program (EAP)
The EAP is provided at no cost to employees and is available to employees and eligible family members. The EAP provides a confidential referral source to assist in dealing with personal, legal or financial problems 24 hours a day, 365 days per year.
